Notes:
(1) Please note that ethical approval must be obtained prior to any data collection or analysis taking place.
(2) Ethical approval is time-limited, normally to be granted initially for one year. Justifications for extension must be provided in the “Amendment Form of an Approved Project”.
(3) The completed application form, together with all related documents, should be submitted online.
(4) No HREC reference number will be assigned to your research ethics application. You will receive a notification email which serves as a proof of your research ethics application being approved by the Departmental Research Ethics Committee. Please be reminded to delete the line about “HREC Reference Number/HREC Approval Expiration Date” in the Informed Consent Form/Debriefing Form.
(5) Please save your file in the format of “UID_YYYYMMDD_Document_Name” (e.g. 3035123456_20211208_Informed_consent_form.pdf / 3035123456_20211208_Informed_consent_form.docx )to facilitate the ethics review process.
